The scope of this notice
The patient access page asks for an email address and, for new patients, a full name, date of birth, and consent acknowledgments. Verification is not connected on this website yet. Entered values stay in the current page’s memory; they are not sent to a server or saved by the application, and refreshing the page clears them. No account is created or email sent by the current form.
This notice is not a healthcare provider’s HIPAA Notice of Privacy Practices. Clinical services require their own appropriate privacy information. The current registration interface does not collect medical histories, payments, insurance information, or medical records.
Information involved in using this site
Hosting and network services may process ordinary technical information needed to deliver and protect the website, such as IP addresses, requested URLs, browser details, timestamps, and error or security logs.
If you choose to email our general support address, your email address and the content you send will be processed by the support and email services used to respond. Please do not send medical records or other sensitive health information.
Cookies, analytics, and your choices
The application does not include advertising pixels, behavioral analytics, third-party marketing trackers, or a cookie-based personalization feature. Fonts and site imagery are served with the site rather than loaded from external font or stock-photo services.

Retention and deletion can depend on the type of information, operational needs, applicable obligations, and service-provider controls. Do not use the website as a place to store information you need to retrieve later.
Changes to this website
The practices described here apply to the current website. Before registration is connected to an account or clinical service, this notice must be updated to explain processing, recipients, retention, and applicable choices. Changes to tracking, payments, or other data practices also require corresponding updates.
